Job Title: Mental Health Case Manager – Registered Nurse (RN)
Location: Virtual Quebec
Employment Type: Full Time Contract Position 6-8 months (With potential to lead to permanent job offer) This is not a part time or casual role.
POSITION OVERVIEW:
We are looking for an experienced and compassionate Mental Health Case Manager – Registered Nurse (RN) to provide exceptional case management services to individuals participating in mental health programs. As a key member of the team, you will ensure a seamless and supportive experience for members, guiding them through their mental health journey. You will coordinate care, provide proactive communication, and work closely with mental health professionals and third-party partners to deliver high-quality mental health services.
QUALIFICATIONS:
- Bachelor’s Degree in Nursing (BScN) or equivalent.
- Licensed to practice in Quebec- Ordre des infirmières et infirmiers du Québec
- Minimum of 2 years’ experience in mental health case management or a related field.
- Strong understanding of mental health conditions and care coordination.
- Excellent communication, organizational, and interpersonal skills.
- Bilingual (French and English) proficiency required.
- Strong technical proficiency with case management systems and telehealth platforms.
- Ability to manage sensitive situations with empathy, professionalism, and confidentiality.
Responsibilities:
- Serve as the case manager for members during their mental health program participation, ensuring the highest quality member experience through proactive communication and case progression updates.
- Conduct comprehensive health assessments using a bio-psycho-social model, gathering relevant data on members’ current conditions and health history.
- Assist psychology and psychiatry experts by contributing to the completion of outcome measures and assessment questionnaires, such as psychological inventories/tests.
- Facilitate the expedited procurement of medical records for members to ensure timely and accurate treatment planning.
- Schedule treatment action plan discussions between members and Teladoc clinicians to review expert recommendations.
- Support members in implementing their treatment plans, identifying the most effective care pathways based on expert recommendations.
- Provide technical and case support to Teladoc clinicians to ensure smooth case progression and resolution.
- Meet defined service level standards for case management, overall quality, and member satisfaction throughout the mental health program.
- Educate and train members, third-party partners, and internal teams on various topics, from administrative tasks such as using video conferencing tools to understanding the mental health program’s processes.
- Assist in building and maintaining a network of quality mental health community resources and partners to support members’ care needs.
- Collaborate with third-party partners to ensure a seamless case management process with measurable outcomes and metrics.
- Perform additional duties as required, with time allocation subject to business needs.
