Description:
Are you interested in an opportunity to develop skills in supporting care home residents with dementia and behaviour that challenges?
This is a rare and fantastic opportunity to join a highly supportive and caring team who work countywide to support residents and staff in Older Adult Residential and Nursing Homes.
CHSS is a clinician led service with consultant supervision available from a Gerontologist and Psychologist
This can be considered as either Fixed Term or Secondment opportun ity Covering a B6 Mental Health role with Care Home Support Service (CHSS)
To work within the CHSS MDT to provide holistic assessment for residents presenting with dementia and behaviour that challenges.
To support care home staff to develop psychosocial care plans to meet residents needs in dementia. This will also involve working alongside families, social services, primary and secondary care colleagues to ensure the resident receives optimal care.
To provide specialist advice, support and consultancy to health professionals, patients, and relatives.
To regularly interface with Older Adults Mental Health Team.

The service works with care home staff in all older adult care homes across the county of Oxfordshire. The aim is to promote the health and well-being of residents through assessment, education, support, and advice, especially for those residents who may be experiencing behavioural and psychological symptoms of dementia.
