REQUIREMENTS OF THE JOB
Undergraduate Degree / Diploma in Human Service or Social Services
Registration with a provincial regulatory body.
Minimum 5 years’ experience working with people who experience emotional and psychological difficulties, preferred geriatric experience.
Experience with various counselling methods.
Experience in recreology in individual and group settings
Excellent verbal and written communication skills
Must be able to provide future planning to clients and caregivers
Excellent team communication and building
Ability to interact with patients/family with diplomacy
Excellent conflict resolution skills
Excellent knowledge of community programs
Ability to work with community stakeholders
Excellent work and disciplinary record
Advanced level in both official languages (oral and written in English; oral in French)
Current certification in Basic Cardiac Life Support and Nonviolent Crisis Intervention
Responsibilities:
The Mental Health Counsellor (MHC) will work with geriatric clients experiencing a wide range of emotional and psychological difficulties to help them bring about effective change and/or enhance their wellbeing. The clients may have issues with depression, anxiety, stress, loss and health decline that are affecting their daily lives.
The MHC will assist the client to develop coping strategies and enhance their resilience.
The MHC will assist clients with recreation/leisure activities for post discharge plan to minimize social isolation risk.
The MHC will develop and implement the group program for patients according to their interests and needs, their physical and mental capabilities, as well as embody a culture of caregiver inclusion in the patient’s plan of care, along with the assistant and input of the Multidisciplinary team.
The MHC will report to the Clinical Manager of Chronic Disease Management
