MS Specialist Nurse

We are seeking a Specialist Band 6 Multiple Sclerosis Community Nurse to join our Huntingdonshire Multiple Sclerosis Service.
You will manage a clinical caseload across Huntingdonshire where your base will Hinchingbrooke Hospital. Delivering evidence-based holistic nursing provision to patients with Multiple Sclerosis working alongside the clinical lead. Within this role you will work collaboratively with the wider MDT, both the Integrated Early Supported Discharge for Stroke and Community Neuro Rehab Team and the wider CPFT services in addition to wider system teams.
This exciting opportunity involves working in different settings, including patient’s homes, clinics, and using virtual consultations to manage your caseload.

You will use your clinical skills and knowledge to shape the service and support others in their development. With the clinical lead you will co-ordinate a specialised nursing service for people and carers of people with Multiple Sclerosis within Huntingdonshire ensuring continuity of care.
You will work closely with all members of the Multidisciplinary Team and provide specialist advice and support to all healthcare professionals within the field of Multiple Sclerosis.
You will be responsible for your own caseload and undertake specialist assessments of patients with multiple sclerosis.

The ideal candidate should be flexible and adaptable to support service delivery.
The successful post holder must be able to travel independently across their work locality, have a valid driving licence and access to a car for work purposes.

• Plan, implement, and evaluate individualised care for people with MS within own caseload, including conducting Annual Reviews, Holistic Reviews, reducing the need for hospital admission under the supervision of the Lead MS Nurse Specialist.
• Be professionally and legally accountable for all aspects of own work, including managing caseload efficiently and effectively.
• Communicate and collaborate with multidisciplinary teams to ensure a coordinated service, representing the MS team at meetings, case conferences, and discharge planning.
• Follow service users and their carers from diagnosis through palliative care, working across health and social care pathways.
• Manage clinics, waiting lists, and ensure high-standard electronic care records on SystmOne.
• Adhere to Trust risk assessment, risk management, and infection control policies and procedures.
• Maintain NMC registration and professional standards.
• Provide patients and their families with information on the standards expected from the team.
