Description:
QUALIFICATIONS:
- Education/Experience:
- Master of Social Work degree (MSW) with training and/or certification in a specialty area preferred (e.g. stroke, chronic disease, rehabilitation, etc.)
- Current registration with the Ontario College of Social Workers and Social Service Workers
- Thorough knowledge of the Code of Ethics and Standards of Practice as set out by the Ontario College of Social Workers and Social Service Workers
- Comprehensive understanding of the social determinants of health of Indigenous people and the social consequences and risk factors associated with client conditions (i.e. addictions, trauma, mental health )
- Experience related to client care for people from Indigenous, stroke, and addictions and mental health populations
- Training and experience in motivational interviewing and solution focused methods, principles in self-management and strengths-based interventions
- Knowledge and experience in establishing networks of support and accessing community resources and services.
Skills/Abilities:
- Ability to manage a complex clinical practice in a health care setting
- Understand the importance of trauma informed practice through a cultural lens
- Knowledge of Indigenous cultures and cultural practices, world view, history, historical trauma, intergenerational trauma, history, impact of colonization, and social determinants of health
- Assessment and treatment of complex psychosocial issues
- Knowledge and skills in a range of theoretical perspectives, psychosocial rehabilitation principles, self-management and holistic wellness approaches
- Ability to communicate effectively and concisely, both orally and in writing
- Ability to establish effective and collaborative working relationships with a variety of stakeholders including community partners within the Indigenous community
- Ability to work independently and collaboratively within an interprofessional team environment with skills, knowledge and attitudes in: interprofessional communication; patient/client/family/community-centred care; role clarification; team functioning; collaborative leadership and interprofessional conflict resolution
- Demonstrated computer proficiency including skills with MS Office applications and electronic medical records
- Strong time management, organizational skills, analytical/critical thinking, problem-solving, and decision making skills
- Ability to use independent judgement and maintain confidentiality of records, actions, and information
- Demonstrated regular attendance at work
