Description:
We recognize and reward top talent. If you are looking for a team environment where you can also make an individual contribution and are seeking opportunities for continual learning/advancement, we encourage you to apply today!
JOB SUMMARY:
The Navigator’s role will be to provide low-barrier care, meeting individuals where they are, when they are willing to consider care. This follows the evidenced-based concept that there is no wrong door as an entry point to services. The Navigator will play a neutral role, welcoming everyone, and therefore avoiding the shame and stigma often associated with seeking mental health or SUD services.
The Navigator will increase access to care for those who seek services at Penny Lane without a referral by providing a same-day screening and appropriate service referral(s). The eligibility and enrollment status will be checked for relevant service programs so individuals will know their eligibility status prior to seeking services. The individual in this position will increase individuals’ access to diverse programs and services. The Navigator link individuals to Penny Lane services or local external programs including, but not limited to, mental health services, substance use disorder services, housing, enhanced care management, benefits, medical care, educational services, pre-vocational services, vocational services, rehabilitative services, or other support services. The Navigator will consult with internal staff and community partners about resources and potential clients.
The Navigator will have full command and understanding of all Penny Lane Center programs, eligibility and availability. The individual in this position is an energetic, self-starter who can function comfortably in a team environment or independently. The Navigator relates well to others and must demonstrate good communication skills, judgement, warmth, compassion and empathy.
This position is primarily office based with some community outreach events, as assigned. It requires flexibility and the ability to manage time and multiple tasks. The individual in this position must be able to assume a wide range of responsibilities, provide timely responses to requests and deadlines, and work well under pressure.
REQUIREMENTS:
