Conduct surgery consultations, telephone consultations, and manage patient queries. Manage a wide range of acute and chronic illnesses, ensuring care is delivered in accordance with evidence-based guidelines and best practices. Make autonomous, professional decisions regarding patient care, whether self-referred or referred by other healthcare professionals within the PCN. Assess and treat patients with undiagnosed and undifferentiated problems, ensuring accurate diagnosis and treatment.
Participate in the management of chronic diseases such as diabetes, hypertension, asthma, and COPD. Work with patients to develop individualised care plans in line with disease management protocols. Screen patients for risk factors and early signs of illness, providing preventive care and health education as necessary. Counsel patients on lifestyle and health management, promoting wellness and disease prevention.
Record clear, accurate, and timely consultation notes in line with agreed standards. Review, authorise, and issue repeat prescriptions in accordance with the practice prescribing formulary. Address queries, complete paperwork, and respond to correspondence in a timely fashion. Share responsibility for home visits and contribute to the PCN’s Enhanced Access Service via a rota system.
Collect data for audit purposes to support continuous improvement and quality care within the practice
