Description:
We have a vacancy in the Night District Nursing Team in Inner NorthWest London covering the boroughs of Hammersmith & Fulham, Kensington and Chelsea and Westminster. We wish to recruit a Band 6 night community staff nurse who is committed, forward thinking and enthusiastic, covering the service which runs from 20.00 to 08.00, 7 nights a week. Your hours of working will be 28.45 hours per week worked over a four week period.
You will be highly motivated and enthusiastic. You will be able to deliver excellent quality nursing care to people in their own homes; work on your own initiative, triaging and prioritising patients and be an excellent team player. We are looking for nurses who are experienced in community nursing and competent in a wide range of clinical skills. The successful candidate will work under the delegation of the District Nurse Team Leaders and will possess excellent communication, assessment and care planning skills. You will work in conjunction with a HCA and visit patients in pairs. You will be fit and mobile, able to walk and manage stairs, carry equipment and move patients with appropriate aids.
Candidate needs to Hold a full, valid, UK driving licence (either automatic or manual) and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010). Candidates must be prepared to safely drive and park our pool cars in Central London at night.
To provide skilled, effective and evidence-based nursing care to patients in a variety of community settings throughout Inner London CLCH.
To be able to work autonomously taking continued responsibility for first assessments and on-going assessment, planning, implementing and evaluation of care.
To actively work with the GP out of hours service, community alarm, community matrons, team leads, LAS and others to identify patients who require a more urgent response.
To prioritise your patient list by assessment of the patient’s needs and by liaising effectively with your colleagues to ensure that those patients requiring more urgent visits are seen promptly.
To provide specialist nursing skills such as, intravenous drug administration, enteral feeding management, palliative care, syringe driver set up and verification of death.
To supervise and co-ordinate the team on a day-to-day basis and to manage the team, with the support of the District Nurse Team Leaders. On-going management responsibilities include staff appraisals, supporting junior staff with their personal development plan and assist in the recruitment and selection of staff and assisting in the performance management of junior staff.
To actively participate in the teaching and support of junior staff, student nurses, support staff and GP trainees.
To actively participate in clinical supervision on a regular basis in line with Trust guidelines.
