Description:

PRIMARY DUTIES AND AREAS OF RESPONSIBILITIES The post holder will focus on assuming a Lead Role in specified areas under the GPN supervisor; · Smoking Cessation clinics · Maintaining Infection Control Standards · Health Promotion advice to patients · Supporting the General Practice Nurse with QOF · Flu planning · Recall Child Health, · Maintaining Safety and Quality · Checking Equipment / Emergency Drugs oxygen etc. · Demonstrate an ability to undertake baseline physical observations and record results correctly. In addition be able to identify anomalies that should be referred to the GPN / ANP or GP Medical staff. · The post holder will contribute to the ongoing assessment of care, undertaking baseline observations of Blood pressure, Pulse, Respirations, Weight, Height, Elimination pattern, Eating pattern, Sleep pattern, Glucose monitoring · Discuss and plan care under the supervision of registered staff, where applicable escalating concerns or complex cases to the GPN/ANP or GP · Demonstrate evidenced based knowledge in the main physical health problems that adult patients may present with i.e., High blood pressure, Diabetes, Obesity, Epilepsy, Respiratory Conditions, Asthma COPD, Chronic Kidney Disease, Cardiovascular Disease, Poor sleep, Constipation, Foot care and Lack of personal hygiene.

  • Demonstrate evidenced based knowledge in Mental problems/ Substance Misuse, which may affect physical health. · Manage situations with patients this may arise from anxiety / emotions relating to poor mental health and or direct them to GPN / ANP/ GP as appropriate. · Articulate an understanding of MDT in and staff roles/ Mental Health, community Nursing 0-19 services etc. · Assess and manage risk associated with the working environment ensuring appropriate BHF policies and procedures are followed accordingly.
  • Demonstrate an ability to use a range of therapeutic techniques when dealing with patients with challenging and unpredictable needs. · Understands the concept of verbal and non-verbal communication and be able use the skills at the desired level. · Will be able to discuss and advise patients, with chronic conditions and carers or relatives. · Will communicate with the patient about care planning and risk assess report to GPN as appropriate.
  • Communicate complex and sensitive information verbally and in writing. · Confidently and accurately convene patient information in the role of an associate nurse in meetings · Has an understanding of risk behaviours and risk management. · To record Incidents in line with local policy procedures and standards. · To communicate risk issues as they arise to ensure safe care.
  • The post holder will engage in positive relationships with staff/ patients/carers/families and others based on respect and within appropriate boundaries. · Act in a professional capacity at all times. · Actively contribute to staff Governance meetings. Improving safety and quality of care: · Nursing associate’s role in maintaining and improving care, will be to support BHF with research and evaluation, quality improvement strategies.
  • The Nursing associate will participate in audit, health and safety activities, as discussed with Head of Nursing · Nursing associate will escalate any risks or safeguarding concerns to the GPN and follow the policy for safeguarding. Contributing to integrated care: · Nursing Associate will refer to agencies as necessary e.g Community Nursing receive referrals to or from, or co-ordinate care with the GP · The post’s responsibilities are to refer for blood tests as delegated by GPN - Tasks relating to these teams or agencies/ wound care / phlebotomy etc. · Responsibilities are the safe transfer of care between professionals, settings or services using SystmOne Responsibility for Information Resources: · Record care undertaken in the SystmOne notes. · Maintain notes that are accurate.
  • Ensure patient records comply with issues relating to confidentiality and safe storage. · Be able to use the NHS.NET email system in line with BHF local policy and procedure.
