Description:
Clinical Demonstrate understanding of all elements of the nursing process and be able to assist the registered nurse in the on-going assessment planning management and evaluation of care Support people with all aspects of care including daily living providing person-centred care and promoting health and independence through awareness raising and care navigation to other professionals as appropriate Perform and record clinical observations including blood pressure temperature respirations pulse and calculate a NEWS score Undertake clinical tasks including venepuncture wound care including compression bandaging and continence care Accurately record nutritional and fluid intake complete screening tools including MUST and Waterlow. Demonstrate understanding of caring for individuals with particular conditions for example dementia mental illness learning disabilities Engage in reflective practice including management of self and reflection on own reactions asking questions and reflecting on answers given Signposting patients to services which may be of benefit to their wellbeing promoting community connections and make best use of local assets. Assist in the delivery of complex care as prescribed by the registered nurse To administer medications in accordance with NMC code Demonstrate knowledge of clinical audit and clinical effectiveness Professional Assist with the implementation and monitoring of clinical standards and outcomes Demonstrate good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures Demonstrate good understanding of the Mental Capacity Act/Deprivation of Liberties and applied principles to everyday practice seeking advice/guidance from the Registered Nurse or registered care professional as required Develop a working knowledge of other providers resources and referral systems to ensure individuals needs are met within parameters of practice Insight into how to evaluate own strengths and development needs, seeking advice where appropriate Time management skills and ability to prioritise Knowledge of when to seek advice and refer to a registered care professional Able to provide and monitor treatment Able to recognise when escalation to a registered practitioner is required Ensure the privacy dignity and safety of people is maintained at all times Report back and share information with the registered nurses on the condition behaviour activity and responses of people Clearly document and evaluate the care given in the electronic care records. Recognise issues relating to safeguarding vulnerable children and adults and report any problems or raise concerns to the appropriate registered care professionals Leadership Demonstrate skills in relation to coaching/teaching individuals/carers/other staff Ability to work on own initiative and delegate work appropriately Ability to take part in reflective practice and clinical supervision activities Demonstrate the ability to recognise changing priorities seeking advice and guidance from the Registered Nurse or other registered care professionals as appropriate Assist in the assessment of and contribute to the management of risk across several areas within the environment where care is being delivered Assist in the implementation of appropriate action to meet the specific physical emotional and psychological social cultural and spiritual needs of individuals and carers Promoting health and preventing ill- health Support people to improve and maintain their mental physical behavioural health and wellbeing.

Working in integrated teams Contribute to the provision of care for people including those with complex needs. Understand the roles of a range of professionals and carers from other organisations and settings who may be participating in the care of a person and their family.

Professional and personal development Maintain responsibility for the identification of own continuing educational needs and development. Reflect on own practice to identify development goals and formulate a Personal Development Plan and take part in annual appraisal. Maintain competence in Resuscitation Moving Handling and Fire training and other Your Healthcare annual mandatory training in adherence to Your Healthcare Policy.
