JOIN OUR TEAM AS A FANAU OLA NURSE CASE MANAGER AND BRING YOUR EXCEPTIONAL CLINICAL LEADERSHIP SKILLS TO THE FOREFRONT FOCUSING ON THE MANAGEMENT OF OUR PACIFIC PATIENTS AND THEIR FAMILIES!

Reporting to the Fanau Ola Nurse Lead you will be will be responsible for providing clinical leadership for the management of Pacific patients with complex health needs that are highly intensive users of health services who are referred to the Pacific Health Development Team for assessment and support within the Hospital or community.
You will actively work in partnership with the medical and nursing staff across the care continuum to ensure safe, patient and family centred care is in line with the vision and values of Counties Manukau Health.
Your primary goal is to enhance the management of Pacific patients and their families by addressing complex health and social issues.
The Nurse Case Manager – Pacific Health Development will be responsible to support individual patient pathways across the primary, secondary and tertiary care interface, establishing effective working relationships with members of the multidisciplinary team, patients, their families and care givers.

To be successful in this role you must possess the following essential qualities:

  • Be a NZ registered nurse with a passion for Pacific health
  • Hold a full and clean driver’s license

Additionally, preferred qualities include:

  • Minimum of 3 years’ experience
  • Fluent in a Pacific language
  • Already functioning at a Proficient or Expert level on the PDRP

Your ability to analyse complex situations and understand diverse clinical perspectives will be crucial in driving positive change.
