Description:

Summary:
The Cardiology Advanced Practitioner provides care to patients with acute and chronic cardiovascular conditions in the outpatient clinic setting. The Cardiology Advanced Practitioner utilizes advanced knowledge to identify, prevent, or solve complex and/or recurring patient care problems. in collaboration with the physician, the Advanced Practitioner manages patients enhancing many aspects of patient care including core measures, patient throughput, and efficient coordination of care after discharge. In addition to direct patient assessment and treatment, the Cardiology Advanced Practitioner provides consultative services, inter and intradepartmental liaison, education, and clinical performance improvement leadership. The Cardiology Advanced Practitioner will incorporate both independent decision-making abilities and collaborate with the physicians, interdisciplinary care team, and leadership to provide expert care to the patients.

Job Responsibilities:

  • Support efficient management of patients with known/potential cardiac diagnoses such as chest pain, syncope, dysrhythmias.
  • Daily physical assessment on all patients.
  • Consult with the supervising physician regarding physical findings and course of treatment.
  • Order appropriate medications as determined by the supervising physician.
  • Order diagnostic tests as determined by the supervising physician.
  • Initiate consultations to other services, authorized by the supervising physician
  • Evaluate and monitor quality measurements as indicated by Steward Health Care Network in accordance with the supervising physicians’ guidelines.
  • Evaluate specific minor illness and injury of patients as defined by approved medical protocols.
  • Collaborate with health care professionals in the planning, implementation and coordination of health teaching and counseling of the patient, family, or patient representative. Document patient level of understanding.
  • May also be responsible for coordination with other clinical staff, nurses, other physicians involved in the care of patients.
  • The APP shall be accountable for maintaining the confidentiality of all Steward Medical Group and patient related information.
  • Analyze pertinent clinical data, communicate findings, and document information appropriately.
  • Collaborates with interdisciplinary care team to facilitate a comprehensive plan of care and coordination of resources.
  • Monitors patient outcomes in accordance with defined standards of patient care.
  • Identifies system-wide factors contributing to undesirable outcomes, such as complications, increased length of stay, and overutilization of resources.
  • Participates in the development of formal and informal educational programs for hospital-based staff.
  • Actively participate in patient safety and clinical process improvement projects.
  • Administer all medications according to hospital policy.
  • Document according to policies and procedures.
  • Adhere to hospital policies and procedures; support the mission of the hospital.
  • Communicate effectively with peers, managers, ancillary departments, and other customers.
  • Other duties as required.

Requirements:

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program required
  • Current board certification from the American Academy of Nurse Practitioners required for NP candidate
  • Current Massachusetts Nurse Practitioner or Physician Assistant license required
  • At least 3-5 years of previous experience working in a dedicated cardiology practice or hospital setting preferred
