DaVita is looking for a Full-Time Nurse Practitioner (NP) Manager to join us in a remote role supporting teams in the Baltimore, D.C. and Alexandria areas (DMV)
The Nurse Practitioner Manager leads and manages a team of Nurse Practitioners by providing education, Risk Adjustment, coaching, engagement, and support.,
In this role, you will provide clinical leadership, oversight, coaching, supervision and onboarding of Nurse Practitioners to ensure that quality, cost-effective care is provided for CKD and ESKD patients.
You will participate in population health management and overall financial viability of the program. This role functions autonomously, yet in partnership with operational and clinical leadership on budgeting, quality initiatives, outcomes and performance measurements.
REQUIRED EXPERIENCE
- Demonstrated experience documenting and coding from an MA (Medicare Advantage) risk perspective and conducting Health Risk Assessments on high-risk/chronically ill patients.
- At least 2 years’ experience in supervision or leadership of Nurse Practitioners
- Documented experience in HCC and Risk Assessments with a focus on chart audits and quality metrics
- Must reside in either the greater Washington DC area or Baltimore area
- 8+ years of NP Experience; experience in population health, Primary/family care, and/or Nephrology Required
- Master’s degree in Nursing.
- Current NP License
- Excellent verbal and written communication skills
- Demonstrated proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation regarding the needs and care of chronic disease
- Deals with confidential information and/or issues using discretion and judgment
- Intermediate computer skills and proficiency in MS Word, Excel, and Outlook
- Excellent organizational skills and attention to detail.
