The Nurse Practitioner reports to the Director of Care with accountability to the Medical Director and collaborates with the resident, family/caregiver, and health care team in the development, implementation and evaluation of the residents’ plan of care, as well as provides leadership and mentorship to Nurse Managers and clinical staff that enhances their knowledge, assessment skills, and ability to care for residents. Leads and collaborates in research, education and evidence-based practice initiatives to optimize the resident, LTCH and health system outcomes.

EDUCATION

The qualifications needed to join our family are as follows:

  • Registered Nurse Practitioner in good standing with the College of Nurses of Ontario, or a Registered Nurse Practitioner registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60
  • Comprehensive knowledge of evidence based clinical practices
  • Comprehensive knowledge of long-term care concepts and philosophies
  • Excellent interpersonal and communication skills
  • Post-graduate education and experience in gerontology (recommended)
  • Specialty certification in gerontological nursing (recommended)
  • Experience working as a Nurse Practitioner,
  • Experience in long-term care, perferred,
  • Experience providing leadership and supervision in any healthcare environment.
  • Ability to embrace various skills from the team to maximize the potential to enhance the well being of all individuals
  • To show evidence of being focused on people’s feelings when interacting and knowing how to turn a task into positive social interaction.
  • To undertake and meet all legal requirements within a context of promoting individuals’ rights and measuring/reducing elements of risk.
  • Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
  • As a condition of employment, candidates are required to submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements
  • Two supervisory references required
  • Provide proof of all required vaccines. The Employer strongly recommends to “stay up to date” with the recommended vaccines. Vaccination requirements are subject to changes as determined by the Employer

Responsibilities:

  • In collaboration with the Medical Director and Attending Physicians, provides comprehensive primary care to residents
  • Collaborates and consults with physicians, the Director of Care, nursing staff, interdisciplinary team members and external resources regarding residents’ plan of care.
  • Assists, supports, guides, and provides consultation to the Director of Care, Nurse Managers, nursing staff and interdisciplinary team members regarding challenging clinical situations.
  • Makes referrals to specialized consultants, services, and other health providers.
  • Performs a person-centered health assessment of residents on admission, annually and as needed (including medication reconciliation and quarterly medication reviews).
  • Conducts person-centered comprehensive health histories and participates in regular care conferences.
  • Orders and/or performs appropriate screening and diagnostic investigations, interpreting results and assuming responsibility for follow-up
  • Identifies and implements research-based innovations for improving resident care and collaborates with members of the interdisciplinary team and/or community to identify research opportunities and to conduct and/or support research
  • Other duties as assigned and or indicated in Job Task Inventory.
