Description:

The Nurse Practitioner Clinic examines and treats patients independently and in autonomous collaboration with other health care professionals following established standards and practices. Ensures proper illness and injury care and disease prevention, diagnosis, treatment, and recovery. Relies on education, experience, professional training and judgement to accomplish responsibilities. A wide degree of creativity and latitude is expected. Works under minimal supervision under the direction of the collaborating physician, as appropriate. May lead and direct the work of others within scope of practice.

  • Participant Care
  • Assists participants in identifying and using community resources available for follow-up health care services. The nurse practitioner shall create and maintain accurate levels with appropriate legal documents and other reports of patient care consistent with the laws of the State of Louisiana.
  • Prepares teaching plans based on nursing needs. Additionally, the practitioner will counsel individuals, families and groups about health and illness and promote health maintenance.
  • Performs health histories, physical exams and patient teaching. Orders, interprets and follows up on lab tests and results. Performs selected procedures and documents care orally and in writing. Initiates emergency measures as indicated. Recognizes abnormalities that justify immediate referral for intervention by others.
  • Performs physical assessments as needed for problems with Participants, that occur on a daily basis.
  • Assures the safe and appropriate use of drugs and monitors the drug therapy of each participant.
  • Partnership and Collaboration
  • Plans, implements and evaluates nursing medical care consistent with medical treatment and care in collaboration with a physician. Additionally, the practitioner will initiate or modify medical treatment when and to the extent authorized by the treating physician within an established plan of medical protocol.
  • Manages primary care or chronic care in collaboration with various health team members such as physicians, nurses, and other support personnel using physician/nurse practitioner established protocols.
  • Quality
  • Delivers primary acute or chronic care focusing on the maintenance, achievement, and restoration of optimal functions of participants.
  • Consistently exhibits a professional manner, appearance, and performance in the approach to job related duties in an effort to maintain high standard delivery of health care. When in contact with Participant, performs all care duties in a manner that demonstrates a humanistic attitude for the physical and psychological needs.
  • Strives to promote the quality and efficiency of his/her own performance by remaining current with the latest trends in field of expertise through participation in job-relevant seminars and workshops, attendance at professional conferences, and affiliations with national and state professional organizations.
  • Participates in interdisciplinary team meetings. Participates in QAPI process.

Experience - 1 year working with the frail elderly
Licensure - Advanced Practice License from Louisiana State Board of Nursing and Certification as Nurse Practitioner by ANCC or AANP and Current Louisiana state RN license and Current BLS Certification (within 6 months of hire)
