Description:
REQUIREMENTS
- BScN and membership in good standing with the College of Nurses of Ontario, as an RN
- At least 3 years of experience working as a Nurse Practitioner in the extended role in a communitybased setting.
- Extensive knowledge of clinical best practices for the care and treatment of clients with complex medical
and co-morbid conditions.
- Excellent communication and relationship building skills
- Commitment to working cooperatively in a community-based setting as part of an interdisciplinary
team; ability to collaborate within and across internal and external systems playing a vital role in the
delivery of services
Responsibilities:
- Move within various community locations as part of the Mobile Health Unit team
- Set up and manage the mobile clinic environment, ensuring all necessary equipment and supplies are
available and properly maintained
- Provide consultation support to on-site chiropodist re diabetic foot ulcers and lower limb wound care
- Works within established care pathways, referrals, guidelines and protocols
- Educate clients about the importance of foot care in diabetes management
- Provide practical advice on daily foot care routines, proper footwear, and preventive measures to avoid
foot complications

Part-time Contract Position
- Participate in community outreach programs and health promotion activities
- Work collaboratively with other healthcare professionals, including doctors, nurses, chiropodists, and
traditional healers, to ensure holistic care
- When appropriate, orders investigations and interprets results of screening and diagnostic laboratory
tests; determines need for diagnostic imaging, and orders and interprets reports.
- Consults with Traditional Healers when indicated and is willing to learn about Traditional approaches to
health and wellbeing.
- Works independently and collaboratively with members of the interdisciplinary team, consulting with
physicians and other health care professionals when client’s condition is beyond the RN(EC) scope of
practice
- Performs other related duties as required.
