Description:
This is a TRAVEL posting, meaning you are required to re-locate full-time for a period of 6 months. Accommodations will be covered by RhynoCare.
RhynoCare is presently seeking an enthusiastic individual to join our high functioning interprofessional team as a Travel Nurse Practitioner (NP) in a Long-term Care (LTC) setting.
KEY QUALIFICATIONS
- College of Nurses of Ontario registration in the Extended Class,
- Bachelor’s degree in Nursing (BScN); Master’s degree in Nursing (MSN) with a Nurse Practitioner specialization preferred.
- Membership with Nurse Practitioners’ Association of Ontario (NPAO) (preferred).
- Has acquired professional liability insurance (with either RNAO or other liability insurance).
- National certification in gerontology is an asset.
- Ability to manage crises in an effectively, professional manner.
- Excellent written skills and report writing for a clinical setting.
- Ability to communicate with clients in a non-judgmental manner that can be effectively understood – practicing active listening skills to understand client needs and coaching of clients as collaborative partners in their health care.
- Basic CPR certification required.
- Must show evidence of valid current Vulnerable Sector Check (VSC)
- Other immunizations as mandated by the Ministry of Long-term Care (MoLTC)
Responsibilities:
- Delivers primary health care to residents in LTC in the management of acute and chronic medical conditions, therapeutic management, health promotion and disease/injury prevention to deliver comprehensive health services
- Provide on-call coverage for duties and schedules as required.
- Perform excellent clinical assessments combined with sound clinical judgement/diagnostic and treatment skills – within full scope of practice as defined by the College of Nurses of Ontario (CNO).
- Provides clinical leadership to LTC team members centered around resident care needs which can include resident admission/discharge, hospital discharge planning, palliative and end-of-life care, pain management, dementia care and responsive behavior management.
- Conducts health assessments by using various tools and techniques based on resident needs.
- Performs a completes health history, including physical, psychosocial, emotional, cultural, and ethnic dimensions of health.
- Performs physical examination and identifies and interprets normal and abnormal findings and orders diagnostic investigations and interprets results using evidence-based clinical reasoning.
- Collaborates and engages with residents and family members to develop person-centered care plans and synthesizes health information using critical inquiry and clinical reasoning to formulate a diagnosis while identifying health risks and states of health/illness.
- Communicates with residents about the health assessment findings and/or diagnosis, including outcomes and prognosis through the application of knowledge of pathophysiology, psychopathology, epidemiology, infectious diseases, behavioural sciences, and family processes when making diagnoses and providing overall therapeutic management.
- Selects appropriate interventions from a range of non-pharmacological and pharmacological interventions to restore or maintain resident functional, physiological, and mental stability to achieve optimal health.
- Promotes safe resident care by mitigating harm and addressing immediate risks for residents and others affected by adverse events and near misses.
- Liaises with discharge planning services for hospitalized residents to ensure a smooth and safe transition back to the LTC home.
- Collaborates with leaders and community health services in initiatives for the development, implementation, and evaluation of systems to promote continuity of resident care across the continuum and collaborates with the leadership team on educational programs for the care team such as participating in professional advisory and leads and supports quality improvement and research initiatives.
- Demonstrate active participation in the CNO Quality Assurance (QA) Program to ensure maintenance of continuing competence to practice.
- Dedicate 80% of the NP’s time towards direct care to residents, and 20% to research and providing education to improve resident care.
- Ability to incorporate knowledge of diversity, cultural safety, and the determinants of health in the assessment, diagnosis and therapeutic management of residents and the evaluation of outcomes and exercise independent judgment and to assess situations and problems efficiently and effectively.
- Leadership skills, working well within an inter-professional team and ability to foster effective working relationships and ability to provide clinical supervision, education, and mentoring for nursing students, medical students, and other learners, including residents/families.
- Ability to build and maintain effective relationships and partnerships with a diverse range of stakeholders as well as work effectively within multidisciplinary teams.
- Strong and effective verbal and written communication skills.
- Ability to work with flexibility for potentially evenings or weekends participates in providing after hours and on-call coverage.
- Knowledge of RNAO Best Practice guidelines and MLTC Regulations and Act.
