Description:

Clinical To be professionally and legally accountable and responsible for all aspects of own work including the management of individual caseload, working within accepted codes of professional conduct and clinical guidelines (where indicated), to minimise clinical risk. To undertake comprehensive assessments and formulate accurate diagnoses and prognoses in a wide range of conditions requiring rehabilitation, recommending the best course of intervention. This will include specialist skills in stroke and neurological conditions. To draw upon clinical reasoning, analytical and communication skills to inform assessments.
This will include the use of highly specialised standardised and non standardised assessment tools (physical and cognitive), observations, questionnaires, multi-agencies/professional information gathering to determine an appropriate treatment plan Utilise physical, cognitive and sensory assessment batteries and techniques requiring sound knowledge of assessments, eye hand co-ordination, dexterity, physical ability (ie manual handling) and task analysis. Formulate and implement occupational therapy treatment plans based on current evidence and clinical reasoning with an understanding of available treatments including client education, skills development/rehabilitation and compensatory techniques or use of medical devices to support an improvement in quality of life and/or client independence. To act as a source of occupational therapy clinical expertise and advice providing clinical and professional leadership within the rehabilitation services and as a resource to colleagues in other teams i.e. social services.
To maintain and monitor own, and others, professional standards in accordance with College of Occupational Therapists guidelines and standards To assess the patients understanding of the treatment proposals to ensure valid informed consent is obtained ensuring practice is within the legal framework for those unable to consent to treatment themselves. To apply knowledge and skills in assessing feasibility of home adaptations, in order to recommend major adaptations to social care colleagues, that will meet clients and families complex health and social care needs. As a member of the multidisciplinary team works to proactively manage people in the community to avoid unnecessary attendances/admissions to hospital and subsequent in patient stays. To identify appropriate care pathways and liaise / co-ordinate and facilitate multi-disciplinary and multi-agency referrals for clients with complex conditions and their families / carers.
Accept clinical responsibility for a designated caseload of patients and to organise this effectively and efficiently with regard to clinical priorities and use of time. To formulate and be responsible for patient care plans carried out by assistant staff within health and social care services. Provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals, to promote understanding of the aims of occupational therapy and to ensure a consistent approach to patient care. To assess for the provision of necessary equipment and/or minor adaptations and check that equipment complies with Health and Safety standards and to legal requirements, as well as to demonstrate the safe use of equipment/adaptations to clients and carers.
Provide teaching and training to other members of the MDT regarding the management of adult patients in the community, outpatient and inpatient setting. To be responsible for maintenance of accurate written patient records within Trust and Service protocols and guidelines. To complete any necessary documentation/reports for other professionals/agencies as required. Service Delivery Regularly participate in working parties contributing to the development of policies and protocols for Adult Services in the community which will impact on all service users.
To provide an Occupational Therapy service in whatever setting is most appropriate to the patients needs during their journey on the community care pathway. Ensure, as Occupational Therapist, that designated staff implement policy and service development changes. Contributes to the development of the community rehabilitation services that supports the delivery of 24hour high quality patient centred care and a 7 day service that is in line with good practice. This may require some weekend and bank holiday work and altered shift patterns in line with service developments and requirements.
Delivers services in line with Better Standards for Health Care and relevant legislation. Along with other clinicians in Adult Services, draw upon specific professional knowledge and skills to establish effective matrix working Support multidisciplinary team working, by facilitating line managed staff to effectively prioritise, plan and manage their caseloads Contribute to the development and implementation of guidelines, protocols and patient care pathways for use across community services. Be responsible for ensuring the effective selection and use of all treatment resources available. Be actively involved in the collection of appropriate data and statistics for the use of the service.
Management Train, supervise and performance manage more junior staff, technical instructors, assistants and students. This will include the use of formal appraisal documents. To cascade information to staff within area of responsibility. To participate in clinical supervision and through the supervisory framework, support other junior occupational therapists in effective decision making with regard to caseload management To attend team briefing meetings and provide input in relation to Occupational Therapy on day to day issues and clinical matters as indicated.
To advise Senior Managers on the changing patterns/needs of the rehabilitation service as necessary. With other members of the Multidisciplinary Team contribute to the prioritisation of referrals to the Rehabilitation Service To promote and develop quality improvements through supervision, audit and other CPD activities as appropriate to the needs of the Occupational Therapy Service by acting as a clinical supervisor/mentor. To be responsible for own personal professional development, and maintaining a CPD portfolio demonstrating awareness of new clinical practices. To act as clinical educator for undergraduate students.
Quality Be responsible for ensuring appropriate risk assessments are undertaken and reported Line Management. Contributes to meeting national and local performance measures contributing to reports as necessary Supports the collection of relevant data sets to demonstrate performance of occupational therapy services against agreed measures. To participate in clinical audit, supporting staff in undertaking audit as required. Contribute towards a culture in which Equality, Diversity and inclusivity is embedded within the organisation.

Internal: Board Lead Short Term Services , Clinical Lead Nurse - Inpatient Services, Senior Therapist, Business Services Manager, Management Accountant, Workforce Governance Project Manager, HR Manager, Head of Therapy and Outpatients, Head of ICT, Integrated Team Managers, GP Provider and all staff within the Provider Services External: Community Care Services and other key Local Authority Staff, Acute hospital colleagues, Voluntary Sector, Patients Forum, other PCTs staff and GPs and their staf
