Rehabilitation Navigator - Adult Specialized Outpatient Program. Please note, this position is open to Physiotherapists, Occupational Therapists, Recreation Therapists, Social Workers, Speech Pathologists, and Psychologists. The Occupational Therapist II - Rehabilitation Navigator will coordinate and facilitate patient rehabilitation with a team of allied health clinicians and ensure the overall management of the level and quality of services received by patients is in accordance with best practice standards. Duties will include ensuring a smooth transition to outpatient services and from outpatients service to community based programs, efficient and timely processing of referrals and requests, the effective management of care based on established performance and outcome indicators; and the optimal and integrated use of outpatient inter professional and community resources, in order to provide quality service for patients and their families in accordance with their goals and based on functional outcomes. Description:
As an Occupational Therapist II, you will be responsible for the coordination of occupational therapy and other allied health clinical services within a program or service area. You will provide supervision of students and team members as assigned. As an Occupational Therapist II, you will be responsible to work both independently and as part of an interdisciplinary team where you will be an integral member, providing leadership to other healthcare professionals. You will be making day-to-day clinical decisions on patient/family needs regarding assessment, intervention, discharge and follow-up. You will make decisions and advise team members on complex clinical cases and provide clinical leadership within the program or service area to ensure safe, efficient and effective client care. You will ensure occupational therapy practice is evidence-based and in alignment with Alberta legislation and regulations, professional standards of practice, code of ethics and AHS policy and guidelines. You will be providing coordination of the day-to-day operations of an assigned service area and monitoring care delivery, identifying service needs and assisting other medical staff with caseload management, completing chart audits and providing follow-up and feedback to staff. You will be participating in program development and operational planning and representing the department on committees or programs. You will be making decisions on staffing level requirements, workflow and caseload priorities in collaboration with managers and other healthcare providers. As assigned, you will carry out the roles of a direct clinical service provider, consultant, educator and/or advocate in your program or service area.
