Description:

To work across the domain of Urgent Care, including the Emergency Department, Acute Medical Unite, Acute Frailty Unit and Same Day Emergency Care (SDEC).
To work with the Multi- disciplinary Team to promote an efficient, effective service for patients and carers.
Work as an autonomous practitioner, both professionally and legally accountable for all aspects of the professional and clinical work.
Focus therapeutic intervention for symptoms and difficulties associated with presenting diagnosis; being guided by recommendations from up-date Nice Guidelines.
To have responsibility for the therapeutic management of patients within the speciality care pathway.
To be responsible for a designated caseload of patients, where you will assess patients, implement and evaluate treatment plans coordinating therapeutic interventions.
you will assist in the organisation, development, promotion and delivery of the Service.
To liaise with health, education and social services, working in partnership with primary and secondary care, social prescribers and other external agencies to ensure the delivery of a high quality.
Make recommendations to clinical team leader for changes to practice, leading the implementation of specific changes as required, and contribute to service guidelines.
To perform advanced holistic triage and assessment of patients with multi-system symptom presentation associated with complex physical, emotional and psychological health
To develop and deliver an individualised treatment programme
Where appropriate to work collaboratively with primary and secondary health care services, third sector services and social prescribing, education and social services, to ensure the delivery of a seamless and high quality service.
To be responsible for a caseload and work without direct supervision. Indirect supervision from the clinical lead will be via regular formal training, 1:1 sessions, peer review and case studies. Access to advice and support from a clinical or team lead and MDT is available
To provide support to MDT and cover for absent colleagues as required.
To undertake all aspects of clinical duties as an autonomous practitioner.
To act as an advocate for patients and their families when necessary.
To carryout risk assessment in line with Trust/National Protocol.
To anticipate care needs and develop individual patient centered intervention and management plans
To ensure patient care and planned therapeutic interventions are supported by available evidence base and experiential knowledge, and are in accordance with Code of Professional conduct.
To keep up to date with professional standards, keep abreast of recent literature and adhere to NICE guidance
