Description:

We are thrilled to announce an opportunity for two passionate and dedicated nurses to join our Open Access Follow Up (OAFU) service. This innovative remote monitoring service supports patients after cancer treatment, empowering them to self-manage and reducing hospital visits while ensuring safe and regular monitoring for cancer recurrence. Currently, this service is primarily offered to Breast, Colorectal, and Prostate patients.

IDEAL CANDIDATE:

Advanced Communication Skills: Ability to effectively communicate with patients and team members.
Holistic Patient Support: Experience in providing comprehensive support and assessments.
Administrative Proficiency: Strong ICT skills, particularly in Excel, and excellent organizational abilities.
Cancer Care Experience: Previous experience working with cancer patients is beneficial but not essential.

Your primary focus will be on helping patients transition from regular clinical follow-up to remote monitoring.
You will conduct holistic needs assessments, support self-management, and deliver health and wellbeing interventions
Additionally, you will assist the admin team with care coordination, complex care planning, referral triage, discharge processes, and helpline support for patients entering the remote monitoring pathway.
To provide high quality, effective, individualised care to all patients that is compassionate and caring.
To support patients health and wellbeing after cancer treatment, with a particular focus on promoting self-management
To work under the direction of the Patient Support Supervisor to deliver the stratified pathways for post cancer treatment
To assess holistic care needs and develop, implement and evaluate them with necessary supervision, signposting or referring patients to the relevant support
To effectively liaise and communicate with primary care services to ensure patients are suitably supported on a stratified follow up pathway
