The Organizational Change Manager is responsible for leading and managing the implementation of complex change initiatives across a mission critical program within Bayshore Home Care Solutions. The role exists to ensure that changes are adopted smoothly and effectively, minimizing disruption, and maximizing business benefits. The Organizational Change Manager works collaboratively with stakeholders at all levels, acting as a coach for senior leaders and executives, and supporting people managers and supervisors.

Location: Mississauga, ON with visits to branches & clinics as required
Duties and Responsibilities
- Apply structured change management methodologies and lead activities to support project and initiative adoption.
- Design, develop, deliver, and manage communication materials for all stakeholders.
- Conduct impact analyses, assess change readiness, and identify key stakeholders.
- Provide input, document training requirements, and aid in the design and delivery of training programs.
- Partner with business leaders and program manager to align change management strategies with business goals and ensure support for change initiatives.
- Facilitate workshops, develop and implement communication plans, and manage potential resistance to change.
- Develop and implement measurement and evaluation plans to track the impact of change initiatives.
- Proactive risk identification and mitigation plans.
- Maintain relationships with stakeholders, fostering a culture of collaboration and continuous improvement.
Knowledge, Skill and Technical Abilities
- In-depth knowledge of change management methodologies (e.g., Prosci ADKAR, Kotter’s 8-Step Change Model).
- Strong project management skills, including prioritization and deadline management.
- Excellent communication and interpersonal skills.
- Analytical and problem-solving abilities, with a focus on risk identification and mitigation.
- Proficiency in MS Office Suite (Word, Excel, PowerPoint), and MS Project.
- Proficiency in data analysis tools (Excel, Power BI, Power Insights).

Job Qualification
Qualifications
Minimum: Bachelor’s Degree
Preferred: Certified Change Management Professional (CCMP), Prosci Change Management Practitioner (CMP); Project Management Professional (PMP), Certified Associate in Project Management (CAPM) or equivalent experience
5-7 years of experience in change management, preferably within a regulated environment (healthcare, finance, government).
Proven track record of leading and managing multiple change initiatives.
Typical office environment, with visits to branches and clinics as required
Ability to travel up to 50% during key program milestones. Must have a valid driver’s license and access to a car if necessary
