Description:
JOB SUMMARY
Outreach workers are responsible for engaging unsheltered people on the street, assessing their needs, identifying appropriate supports, advocating for their interests, and developing trusting relationships. Outreach workers will work to address basic needs, such as food, clothing, emergency shelter, transportation and identification. Additionally, outreach workers will support individuals with their long-term goals, including housing stability, employment, and health care. Outreach workers will coordinate their efforts with businesses, city agencies and other service providers, and document engagements, services and placements in city databases and other record keeping systems as required.
MINIMUM QUALIFICATIONS
- High School Diploma or Equivalent
- Good computer skills with knowledge of databases and Microsoft Word Suite
- Knowledge of resources available in the housing and homelessness system in Philadelphia
- Experience in and knowledge of homeless programs and general issues around homelessness; experience with mental health and substance abuse issues
PHYSICAL REQUIREMENTS
- Requires extensive walking, sitting, talking in unusual times and circumstances such as early morning and late evening on sidewalks, park benches, alleys, subways; during snowstorms and heat waves; supporting intoxicated or disabled individuals; carrying blankets, coolers, and other equipment to and from vehicles.

Responsibilities:
ESSENTIAL DUTIES AND RESPONSIBILITIES
The Ambassadors of Hope Outreach worker will engage people experiencing homelessness with the objective of developing trusting relationships. Essential to the position will be working with individuals towards long-term stability through housing, employment, health care, and other services. The Outreach worker will additionally work with individuals to address their basic needs, such as connecting them with resources related to food, clothing, showers, laundry, transportation, and identification. The outreach worker will also provide the Center City District and internal partners with the information and resources needed to have more productive interactions with customers in crisis.
The Outreach worker will walk assigned route paying special attention to all ?hot spots?. The Outreach worker will also provide information and resources to more effectively interact with customers experiencing homelessness. The Outreach worker is responsible for utilizing email summaries and databases to record data in a timely and thorough manner.
OVERVIEW OF RESPONSIBILITIES
- Complete entirety of expected routes on a daily basis.
- Attend regular focus list and case review meetings with team members.
- Act as an outreach liaison by interacting with the public at large including visitors, residents, other staff and co-workers in a professional and welcoming manner.
- Complete internal daily shift reporting, utilizing: Outlook, Web focus & ETO.
- Complete necessary follow-up for all individuals placed within internal or external service providers.
- Communicate clearly and consistently with leadership.
- Complete all data & reporting requirements, including individual assessment of each location.
- Through relationship building, collect narrative examples of SCC success and programmatic challenges.
- Act in alignment with Project Home’s vision and mission.
- Observe confidentiality, privacy, and dignity of each resident.
