Description:

An opportunity has arisen for an enthusiastic, self-motivated dietitian to join our Paediatric dietitians providing expert nutrition and dietetic services to Sheffield Children’s NHS Foundation Trust(SCHNFT). This position , offers an excellent opportunity for a dietitian looking to advance their career within Paediatrics and work with Paediatric dietitians and dietetic assistants.
Ideally we want somebody with clinical dietetic experience of working within Paediatrics. However for an experienced band 5 dietitian who can demonstrate the relevant skills and knowledge required we could consider a developmental pathway to a band 6 (on completion of specified competencies and training).
The successful individual will be responsible for a varied Paediatric caseload which will include covering the acute Paediatric wards and undertaking general outpatient clinics. The caseload includes conditions such as- autism, allergies, faltering growth, restricted eating, and nutrition support.

Responsible for a clinical caseload providing specialist advice to patients/clients and carers within the hospital (inpatients and outpatients) and with telephone consultations to the home. Provides advice to patients/clients, carers, the multidisciplinary teams and other healthcare professionals:
Determine/diagnose and prescribe the appropriate dietetic treatment and develop effective treatment plans.
Undertake dietetic assessment and calculate nutritional requirements of referred patients, taking into account medical, social and cultural factors.
Provide nutrition and dietary advice suitable to the patients’ medical, social and cultural factors, which is based on current evidence/ best practise, incorporating professional and national guidelines and standards where available.
Negotiate change with patients to enable them to achieve targets, overcoming barriers to change including dietary, environmental and social factors.
Monitor and review dietetic care plans against outcomes and adjust care plan to facilitate achievement of goals
Ensure dietetic interventions can be implemented through continued collaborative interdisciplinary working by attending and participating in multidisciplinary clinical meetings, ward rounds, case conferences, discharge planning.
Advise on the suitability of food provision for inpatients, working in partnership with services in the provision of nutritionally adequate normal and therapeutic diets. Provide appropriate instructions and advice to staff of the Special Feed unit and to Catering staff.
Ensure up to date and accurate records of all assessments and interventions are noted in medical, nursing and dietetic documentation in line with department, trust and government directives
Ensure safe and timely discharge of patients by providing relevant information to key professionals and agencies and following relevant procedures.
Ensure provision of specialist feeds and nutritional supplements for outpatients by liaising with GPS and other relevant agencies.
Cover staff in absences relevant to grade with supervision.
Educate groups of healthcare professionals, nursing staff, patients and patients carers.
Maintain professional standards in patient management and dietetic treatment
Adhere to trust policies
Maintain good interpersonal skills with staff and colleagues to ensure no harassment, bullying or discriminatory behaviours occur
Undertake PDR and develop personal development plan according to trust policy.
Contribute to meetings and working groups within the department, multi-disciplinary teams and elsewhere.
Record and submit clinical activity data using manual and electronic means according to departmental procedures.
Promote health and safety of patients and staff: report any risks/incidents in accordance with trust risk management policy.
Report to the Dietetic Manager on affairs relating to the job role, highlighting issues necessitating help and guidance and determining progress
To assist the student training dietetic lead in development and management of the ABC
Develop, implement and evaluate high quality evidence-based education in all media to colleagues and other healthcare professionals both formally and informally, at undergraduate or postgraduate level
Use different models of teaching and develop or adapt these in response to needs, in order to facilitate effective learning.
Adapt training and resources to meet the needs of those with communication and psychological difficulties.
Undertake clinical audit using appropriate methodology and present results to promote evidence-based practice.
Undertake critical appraisal of any new evidence pertaining to designated area of practice and to disseminate this within the service and with wider professional bodies as appropriate.
Assist in the development of evidence / good practice based policies on matters relating to nutrition and dietetics and trust wide implementation including national benchmark relating to specialist area of practise
Participate in any research studies following agreed protocols and ethics.
Establish and maintain effective communication with individuals, and groups about difficult or complex matters.
Use counselling skills to establish rapport and a patient/family centred relationship in order to identify barriers to understanding and implications of change for staff and patients.
Use empathic, motivational, negotiating skills in formulating a plan, establishing change and maintaining change for staff and patients
Obtains consent and explains the implication of a diagnosis in relation to dietary needs and long term impact on lifestyle and chronic disease management.
Develops collaborative interdisciplinary working through effective communication across health and social care boundaries.
Uses a range of communication methods (interpreters, pictorial resources) to promote understanding.
Adapts resources to meet language, learning, and physical disabilities associated with communication difficulties.
Formulates and makes presentations to other professionals.
