Description:
To ensure that children and young people achieve a timely and efficient discharge To provide administrative support to the ward managers, nursing and medical teams to support a timely discharge for patients To work closely with the ward managers/nurses in charge, supporting the daily discharge profile for each area and the forward look of discharges, including planning for weekend discharges. To ensure all patients have clear plans in place. Ensuring discharge dates are recorded, reviewed and updated daily in collaboration with the medical teams Attend the Doctors handover meetings and report back to senior teams as necessary. To provide a visible presence and be available for support and direct assistance to wards and departments as required To ensure that all relevant paperwork/documentation and referrals are completed and in a timely manner To trouble shoot avoidable delays in regards to systems and processes related to diagnostic tests/results etc.
