Description:
To help assess the social, developmental and educational needs of individuals and groups of young people and deliver appropriate programmes and activities. To establish relationships with key individuals and, as appropriate, parents/carers and siblings. To work alongside the NUH Youth Forum to create opportunities for young people in CEW Services to participate in service development and actively share their views within the Trust. To support the management of the transition of young CEW patients between paediatric and adults units, using tools such as The Aspire Programme.
To support young peoples development using accreditations such as The AQA Unit Award Scheme. To encourage young people to self-advocate or advocate on their behalf; working closely with relevant services such as PALS. To participate in multidisciplinary meetings, reviews etc. providing professional advice on young peoples issues, reporting any relevant information or safeguarding concerns as necessary, making referrals and maintaining appropriate links.
To support young people in their community, if necessary, to help re-establish their normal routines e.g. returning to school. Enable young people to access appropriate community support services such as health, social, educational or legal services. To establish contact and relationships with groups and organisations to help establish young people in their local communities.
Ensure up to date evidence based practice by close liaison with health-based youth work teams and maintaining an awareness of the national youth work agenda
