Are you a dynamic, highly motivated, compassionate and enthusiastic nurse with a high level of knowledge and skills in palliative care?
If so, an exciting opportunity has arisen for a part-time (30 hours) Band 6 Palliative Care Acute Nurse to join the busy Specialist Palliative Care Team (SPCT) in Pembrokeshire. You will be instrumental in delivering first-class palliative and end of life advice, support and care within the acute setting.
The post holder will possess exceptional communication skills and emotional intelligence, being able to skilfully and sensitively communicate with patients, families/care givers and allied health professionals.
You will join a team of dedicated Clinical Nurse Specialists, Consultant/Speciality Doctor, Therapists and the wider community MDT in providing expert, high quality patient focused care.
You will support hospital teams in the identification of patients with palliative care needs at an early stage in order to prevent avoidable delays in treatment and work with the Consultant and community team to expedite discharge into their preferred place of care.
Have responsibility for a defined and delegated caseload of patients under the guidance of the specialist palliative care clinical nurse specialists and consultant. Support community and hospital staff to ensure that that all patients have a clearly documented and accurate plan of care which reflects the assessment undertaken and incorporates the issues and recommendations made, including advance care decisions and do not attempt resuscitation discussions. Ensure patients are given opportunity to discuss advance care planning openly and that their wishes and preferences are respected and care is co-ordinated. Ensure patients receive the relevant information to enable them and their relatives to make an informed choice about their treatment and future care. Facilitate effective and safe transfer from hospital to enable continuity of the care plan beyond transfer of care.
To act as a point of resource and information. To help develop and deliver teaching programmes for relevant staff/teams. To act as a role model, to encourage staff development. To be able to manage complex or sensitive situations using effective communication and negotiating skills. Communicating difficult news, e.g, relating to prognosis. Dealing with challenging and difficult issues relating to prognosis and future care needs. Provide emotional support to families, relatives and staff in distressing and emotional situations.

  • Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
