Description:

The post holder will work in collaboration with Nimbus Care & St Leonards Hospice to support patients living with frailty, who would benefit from advance care planning and the choice to die at home. Employed by St Leonards Hospice and based within the Frailty Hub, the post holder will provide a high standard of skilled, compassionate nursing care for patients living with frailty who may be reaching the end of life. The role will support the Frailty MDT to assess patients considered to be in the last days/ short weeks of life to deliver end of life care in the patients home. Collaboration and education will empower all staff working with the frailty population to confidently discuss and complete advance care plans and therefore support the choice to die at home.
Main duties and responsibilities Professional To assess, plan and evaluate nursing care, maintaining accurate, legible documentation and communicate necessary information to all relevant team members. 2. To develop, share and maintain expertise in palliative nursing within the Frailty Hub and wider community setting. 3.
To maintain and support the delivery of high standards of palliative care through effective collaboration with system partners. 4. To prescribe in line with the scope of professional practice, where a prescriber qualification is applicable. 5.
To participate fully in the setting, implementation and monitoring of formal standards of practice and be responsible for Clinical Governance and audit for the service. Leadership 1. To provide professional leadership for the service, taking an active role in the development of the service. 2.
To identify and promote good practices in palliative care with the development and regular review of operational policies and procedures. 3. To maintain effective communication within the Hospices Executive Team and Frailty Hub. 4.
To promote a supportive working environment for all staff. 5. To work in conjunction with the wider health community maintaining accurate record keeping with regard to the administration and storage of drugs within the community setting whilst adhering at all times to Hospice and community policy. 6.
To develop and maintain close professional links between the Hospice, local GPs, Macmillan Nurses, Consultants, Yorkshire Ambulance Service, Hospital services, Primary Healthcare teams and Social Services. 7. To support the clinical audit process liaising closely with Hospices Executive Team and Frailty Hub management. 8.
To assist in maintaining and enhancing safe practice within the community setting working in conjunction with fellow community teams with particular emphasis on equipment and working practices, ensuring safety policies and procedures are adhered to. 9. To report all accidents, incidents, complaints and untoward occurrences and keep statutory records as required. Education 1.
To be responsible for maintaining own professional development and sharing knowledge with other members of the team. 2. To assist in the provision of education, support and training of the wider workforce in conjunction with existing workforce educators (palliative care/ frailty). 3.
To teach as required on internal/external study days/courses
