ABOUT YOU:

We are looking for a highly motivated person with experience and a passion for making a difference within palliative and end of life care across north Derbyshire. The successful person will be joining a dynamic, well-established team, that is forward thinking and works collaboratively in an environment thriving on positive change.

Skills and experience required:

  • NMC registration, with post registration experience at Band 5 or above.
  • Level 6 degree in palliative care
  • Advanced pain and symptom management CPD
  • Advanced Communication Skills Training (or equivalent)
  • Experience in relevant clinical setting such as palliative care, oncology, LTC and Community.

Responsibilities:

Hours: 37.5
Working pattern is Monday to Sunday, this will include weekend working (approximately every 6-8 weeks) and bank holiday working. A Rota is produced approximately 2 months in advance.
The Community Palliative Care Specialist Nurse Team is a well-established team providing specialist support to patients in the community and their own homes across north Derbyshire. The team consists of Palliative Care Specialist Nurses, Palliative Care Nurses and Palliative Care Support Workers. The team works closely with the Ashgate Multidisciplinary Team and wider system teams across north Derbyshire.
The team is an integral part of Specialist Palliative care services at Ashgate Hospice.

The main responsibilities include:

  • The management of patients as an autonomous practitioner and where qualified, using Advanced Physical Assessment and Consultation Skills (APACS) and Non-Medical Prescribing (NMP)
  • Providing a focus on quality by delivering evidence-based, individualised and personalised care
  • Demonstrating excellent advanced communication skills.
  • Acts as a role model and provideclinical leadership to others.
