Description:
Catholic Charities of the Archdiocese of Newark is one of New Jersey’s largest non-profit social service agencies with programs and facilities serving Bergen, Essex, Hudson, and Union counties.
We are currently seeking a Part-Time Case Manager for our St. Rocco’s Family Shelter, located in Newark, NJ. St. Rocco’s is a 59-bed emergency shelter that ensures safety and builds stability in the lives of homeless women and their children.
Schedule: Monday through Friday 4pm – 8pm
JOB REQUIREMENTS
- Bachelor’s degree (in social work or related field preferred.
- 2-3 years experience working with the homeless population or related experience required.

Responsibilities:
- Complete intakes, assessments, admissions and discharges
- Develop & implement Family Service Plans and homeless prevention services
- Complete related documentation and progress reports.
- Develop and maintain compliance with HUD, County and DCA essential services standards and related policy and procedures.
- Maintain referrals and communication with County Coordinated Assessment Agency and Head Start and child care providers.
- Develop Housing & Service Plan and provide assistant and linkages in obtaining needed services and achieving the family’s goals.
- Responsible for establishing a case record for each family including Service Plan, progress notes, monthly report, HMIS, and eligibility documents and all other required program documentation.
- Make and follow-up on appropriate referrals as well as provide advocacy for families.
- Conduct screening interviews for prospective program families and intake interviews for newly admitted families.
- Conduct ongoing supportive counseling sessions with families to continually assess needs and progress.
- Collaborate with other team members involved with specific families to provide continuity and consistent care.
- Facilitate groups to increase knowledge and provide support to families.
- Provide crisis intervention and assist in crisis situations.
- Attend trainings, staff meetings, program activities, represent the program at community/providers’ meetings as determined by supervisory staff.
