Pathway Co-ordinator Sleep & Respiratory

The post holder will coordinate the administrative pathway of patients to ensure that waiting time targets are not breached. The post holder will be the forefront of the sleep service liaising with consultants and the physiology team to ensure diagnostics are booked, reported and treatment is actioned as necessary. The post-holder is responsible for the collection of accurate and timely data for those patients on active treatment, awaiting treatment and those that are close to breaching their treatment targets. The post-holder is also to be involved in facilitating service improvement to streamline the patient journey and ensure that the trust delivers a quality service and gold standard patient care.
