Description:

University of Pittsburgh Physicians is hiring a full-time Patient Access Navigator to support UPMC Mercy Vision Institute located at1622 Locust Street Pittsburgh, PA 15219. This position will work Monday through Friday (1 Saturday a year to support the Mission of Mercy). Travel to satellite offices may be required based on the need of the patients. This role offers potential for remote work

Responsibilities:

PURPOSE:

The Patient Access Navigator is an advanced patient services role that will be utilized in a dual capacity. Approximately 75% of this staff member’s time will be functioning as a patient navigator, assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. Approximately 25% of this staff member’s time will supporting the care coordination of patients with retinal dystrophies and other complex cases, including responding to patient calls and messages, scheduling patients, and ensuring appropriate patient follow-up. In both capacities, the staff member will maintain a database with patient-specific information relevant to diagnosis, treatment, follow-up diagnostic and demographic information.

RESPONSIBILITIES:

  • Tasks related to patient navigation include, but are not limited to: ? Initiate communication with patients following discharge from the Emergency Department. Communicate with Discharge Planner/Discharge Manager ? Schedule patients as appropriate for follow-up with a Primary Care Physician (PCP) or specialty providers? Guide patients through the health care system; help patients arrive at scheduled appointments on time and prepared. Secure/Set-up transportation to and from medical appointments? Work collaboratively with other interdisciplinary teams and community partners to accomplish task? Facilitate interaction and handoff communication with other staff and departments? Establish educational sessions to inform physicians, patient service representatives, and all other staff regarding the Patient Access Coordinator role, services provided and the referral process? Assist patients with enrolling in transportation services in their County? Assist patients with enrolling in PENNIE, State Funded Insurance and financial assistance programs? Track interventions and outcomes? Participate in Mission of Mercy Pittsburgh? Participate in Community Outreach Committee Meetings, Resource Focus Group, Healthy Vision Lab? Take monthly inventory of the Mercy Vision Institute clothing closet. Replenish items as needed? Travel to satellite offices dependent upon patients? needs
  • Tasks related to care coordination of patients with retinal dystrophies and other complex cases, include but are not limited to: ? Facilitating new referrals (i.e. intake calls, requesting records)? Scheduling follow-up appointments? Monitoring service email address and phone #? Assisting in data entry and maintenance of the Rare Ocular Disease database? Collaboration with UPMC Global Care
  • General? Utilize interventions and strategies that are appropriate to the population(consider culture, language, age, gender, and special needs? Demonstrate professional behavior, maintain respect for confidentiality and freedom of choice? Perform in accordance with system-wide competencies/behaviors? Performs other duties as assigned
  • Bachelor’s Degree in healthcare, nursing, or related field
  • Two years of healthcare experience with knowledge of patient access and insurance payers required
  • Experience in healthcare with demonstrated integrity, stability, judgement, resourcefulness and self-direction
  • Ability to communicate verbally and in writing with staff, patients and interdepartmentally
  • Ability to organize work, prioritize task, effective team building, excellent customer service skills, problem solve, ability to learn task quickly and efficiently, demonstrate excellent listening skills and patience
  • Ability to exhibit calm communication when speaking with patients
  • Proficient knowledge of medical terminology and Microsoft Office computer applications
