SUMMARY:

The Patient Care Assistant II assists patient care unit staff with delegated tasks of direct and indirect patient care, which support nursing practice under the direct supervision of a registered nurse; carries out designated activities that are within those functions limited by law to unlicensed nursing personnel.

Responsibilities:

May perform various duties, depending on location/unit, to include: Performs patient care with respect for the patient’s dignity, uniqueness, personal preference and rights; communicates acceptance, caring and empathy to the patient/family; ensures patient right to privacy when providing care. Assists the nursing staff as delegated in performing patient care activities including bathing, linen changes, oral hygiene, and comfort measures; assists patients with menu selection, meal set-up, feeding, oral fluids, and records the intake/nourishments; delivers and picks up dietary trays and dietary supplements; applies hot and cold treatments; assists patients with elimination needs; assists with patient activity including disconnecting of drainage tubes from suction devices and unplugging IV pump/controllers from electric outlets, when appropriate; collects non-sterile urine, stool, and sputum specimens and delivers sterile or non-sterile specimens to appropriate collection point; provides assistance in dressing patients, gathering patient’s belongings in anticipation of discharge or transfer; transports/escorts discharged patients when necessary; assists with transport of patients to diagnostic tests and procedures when appropriate; applies abdominal binders, support stockings, and orthopedic appliance/equipment when appropriate; performs care for patients in isolation; assists with postmortem care; orients patient to the nurse call system and surrounding environment; assists patients in using the incentive spirometer; answers patient call lights and responds in a timely manner; rounds hourly for patient repositioning, safety checks regarding environment, pain, position, needs, etc.; assists with blood transporting; assists with applying telemetry monitors on patients; provides assistance with Pressure Ulcer Prevention with high risk patients. Applies ace bandages and splints; assists with medicated baths/showers and application of selected dermatologic smears and wet dressings; lifts equipment; sets up sterile trays, lumbar puncture; assists nurses with large wound dressing changes; returns patient to continuous passive motion device; collects specimen and performs point of care testing; performs bladder scanning, EKGs, IV removal, Foley removal and care, Phlebotomy, use of restraints; collects midstream urine specimens; ensures Oxygen tank safety. Collects data and communicates patient/family concerns to the appropriate RN; measures vital signs including blood pressure, temperature, pulse, oximetry readings, and respirations; observes drainage and empties, measures output; reports pertinent data regarding observations of the patient to the patient’s assigned nurses in established time frame; assists with admissions by obtaining height, weight, vital signs, and by reporting other pertinent data to the nursing staff; collects daily weights as applicable; documents patient care given and records patient data obtained (e.g., vital signs, weight, intake and output) in the patient medical record in a timely manner; communicate to RN at the beginning of the shift the plan of care for the team assigned. Maintains clean, safe patient care area and staff work area by performing activities including organizes environment of the patient care unit (e.g., lounge, nutrition areas) and patient room; cleans equipment used in patient care; empties wastebaskets and linen hampers; assists with room cleaning upon discharge or transfer; cleans showers/tubs after each patient use as it pertains to patient safety; delivers “red bag” biohazardous waste to designated disposal area. Assists with assuring the availability of the equipment and materials necessary for providing patient care; demonstrates an awareness of the cost of prudent use of supplies; obtains medical supplies/equipment; assists with tracking of patient care equipment specific for the patient care unit; reports equipment in need of repair; monitors and maintains clean linen supply; restocks supplies in patient room according to unit specifications; assists with retrieval and delivery of specified items to and from appropriate locations. Assists with other activities on the patient care unit which includes answering communication devices; greeting and assisting patients, family, visitors, and staff; delivering mail; delivering messages; assisting with placement of appropriate signage/information/isolation caddies; attends and/or stays informed of unit activities by attending unit meetings or reading minutes and asking for clarification as needed; updating the patient white boards.
