Patient Flow Co-ordinator- Transfer of care Team

An exciting opportunity has arisen to join the Transfer of care Team at Lister Hospital as a Patient Flow Co-ordinator. This is an important role supporting the team in an operational and administrative Role.
The primary focus of the role is to ensure flow is created in the trust, to allow patients to be placed in the right bed at the right time, supporting patient flow and facilitating timely safe discharges.

As a Patient flow Co-ordinator you will manage the live Discharge today list and ensure nerve centre system is updated.
Participate in transferring patients to the Discharge Lounge, as early as possible, and throughout the day.
Manage and support the home today list on Nerve centre and ensuring patients are discharged as planned.
Communicate and close working with support services, Transport, pharmacy, discharge lounge, Transfer of care Team, Site Office and Ward staff as required.
Ensure that the next day discharges are shared with all support services on Nerve centre application and in real time.
Participate and support full capacity protocol when activated.
Support the wards with booking transport 24 hours in advance to facilitate timely discharges.
