This post is an exciting opportunity to work on our AAU and allows the applicant to be part of an exciting and rewarding challenge, working within a role that can have a direct impact on the organisation of a very busy unit and ultimately improve patient care.
The successful candidates shift pattern will be Monday to Friday 10am whilst 18:00pm.
The ideal candidate would be highly organised with the ability to multi-task and prioritise workload, have the ability to work under pressure and work as team member. Experience of working with system one or a complex NHS patient management system would be beneficial. What we can offer the applicant is an extensive training package and ongoing professional support.
The role provides a unique opportunity to be exposed to multi systems within the NHS, providing a good platform for professional development.
Acute Assessment Unit (AAU) The Acute Assessment Unit (AAU) cares for patients who require an urgent acute assessment and short stay. The AAU works collaboratively within the Urgent Care division to provide treatment to the acutely ill patient.
The primary focus of the flow co-ordinator is to enable and support the safe and timely facilitation of beds for patients requiring assessment on AAU through supporting effective smooth communication between ED, bed manager, wards, and multi-agents within the unit and external.
To work with Airedale NHS colleagues, AAU nursing, medical, social partners, voluntary sector, care/nursing/residential homes, patients and their carers to proactively support and facilitate timely and safe discharges from hospital to home or onward care settings.
The duties and responsibilities listed below are not exhaustive, and the post holder will be expected to adopt a flexible approach when the needs of the service require, and may need to adopt new skills which they will be fully supported in terms of training and time.
