Description:

Job Overview:
Serves as a resource and provides linkage to care for BHMG patients referred by their doctors to Specialists Diagnostics Imaging Physical Therapy etc. therapies and other services directed by the physicians. Serves as a resource and liaison between BHMG Physicians Practices and the Physician Medical Neighborhood Specialists and Hospital-based Physicians. Completes a monthly dashboard and Stoplight Report shares the same with the Physicians and Practice Manager Proposes opportunities for Linkage to Care patient access improvement.
Qualifications:

Required:

  • High School Diploma or GED
  • Experience working in a physician practice with working knowledge of physician referrals insurance requirements and patient scheduling
  • Successful completion of onboarding orientation to BHMG practices and SBMC outpatient services hospital-based physician panels Specialist panels

Preferred:

  • Additional clinical and practice management certifications

Scheduling Requirements:

  • Shift working, 8:30am - 5:00pm
  • Monday - Friday
  • Full-Time

Essential Functions:

  • Increase compliance with referrals to network physicians and services to 75 percent
  • Follow up at 100 percent with patients following the referral/scheduled appointment
  • Follow-up as needed to improve compliance with post-visit reports
  • Maintain and validate the tracking of access through a spreadsheet and report out on a monthly basis
  • Schedule patient appointments for outpatient services and physician panels for designated practices
  • Round regularly on the physicians and leading office staff to receive and address patient and physician feedback
  • Provides a compassionate and supportive environment with prompt and consistent service
  • Other duties as assigned
