Responsibilities:
- Increase compliance with referrals to network physicians and services to 75%
- Follow-up at 100% with patients following the referral/scheduled appointment
- Follow-up as needed to improve compliance with post visit reports
- Maintain a Stoplight Report with issues identified and share with BHMG Manager (Martha Eid)/Physicians
- Maintain and validate the tracking of access through a spreadsheet and report out on a monthly basis
- Schedule patient appointments for outpatient services and physician panels for designated practices
- Round regularly on the physicians and leading office staff to receive and address patient and physician feedbac
