Description:

To assist in the provision of an efficient and effective service to ensure the smooth transfer of patients to and from the Acute Trust to Community Hospital, Intermediate Care Homes and other Health facilities.
To provide general clerical support for Patient Transport Services.
To assist the Patient Transport Services Supervisor within the Trust.
Receive, assess, co-ordinate and advise on requests for Patient Transport. This may involve calls from individual patients / relatives / carers in respect of their transport arrangements.
Where patients meet eligibility criteria, organise the appropriate transport to transfer patients to Community Hospitals, Intermediate Care Homes and other NHS health facilities, as well as private addresses, in liaison with TAP (Transport Access for Patients), SWAST (South West Ambulance Trust) and Private Ambulance companies.
Where the patient does not meet eligibility criteria, advise appropriate alternatives.
To maintain database records of all transport requests.
Implementing the NHS Kernow CCG Patient Transport Policy, with particular attention to the medical criteria, and the subsequent education and training of healthcare professionals.
Use the above to assigning the correct level of care for transportation and where appropriate alter booked mobility of patients and arrange most suitable form of transport.
To co-ordinate the workloads of all private crews, including identifying the number of crews available on any given day, their geographical location and type of vehicle e.g. sitting / stretcher and wheelchair facilities.
To log and verify all transport invoices.
To input non-emergency ambulance bookings and assist with other tasks as appropriate.
