You will have the following skills and experience:

Essential:

  • Adaptable, flexible and friendly with strong administration and organisational skills.
  • Excellent communicator to staff at all levels in all circumstances who work in a confidential and discreet manner and are able to think on their feet in a busy working environment.
  • Able to multi-task, effectively manage own time/workload and work hard to maintain a high level of attention to detail when under pressure to meet deadlines.
  • Able to organise own tasks and work systematically demonstrating skills and activities to others who are undertaking similar work.
  • Able to work as part of a multi-disciplinary team, as well as being self-motivated and feeling comfortable to work independently and having a can-do attitude.
  • Competent in the use of all Microsoft Office platforms.
  • Able to work remotely and on site with the capability of travelling between our 7 practices as required.

Desirable:

  • Experience in a patient facing role in primary care.
  • Knowledge of SystmOne.
  • Experience with employee life cycle and general HR.

ABOUT US

Benfleet PCN was formed in June 2019 with seven like-minded practices wanting to provide a joined up integrated service for our population. The PCN now includes 40 staff members, and we have been known as the accelerator PCN in Mid and South Essex. The PCN provides services for just under 48,000 patients across the patch.

Responsibilities:

Benfleet Primary Care Network has an opportunity for a care co-ordinator to join our PCN, on a full time basis for 37.5 hours per week, to support our 7 GP practices and assist in re-shaping primary care across our communities.
This role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN, and is pivotal in ensuring that all patients receive the best possible care and service.

The main duties of the role include:

  • Support the clinical directors in the delivery of the DES specifications.
  • Answer queries, making, and managing appointments.
  • Contribute to public health campaigns (e.g., flu clinics) through advice or direct care.
  • Provide cross-cover to our other PCN care coordinators at times of annual leave and sickness.
  • Undertake a wide range of administrative duties.
  • Assist the PCN manager with their relationships and engagement with the PCN practices and other stakeholders across the locality.
  • Assist the PCN manager in ensuring the dissemination of information across the member practices.
  • Support in the delivery of training to PCN staff.
  • Support all member practices with data quality issues relating to PCN contracts, enhanced services and IIF/QoF.
