The successful candidate will have the following qualifications, skills and experience;
Qualifications:

Desirable:

  • Qualified from appropriate training course as set out by the Personalised Care Institute for Care Co-ordinators.

Or

  • NVQ Level 3 in Adult Care.
  • Additional Qualification in health or social care allied profession.
  • Long term conditions and welfare rights basic training.
  • Clinical skills and qualification - Phlebotomy, BMI- relevant training and refresher courses can be available for the right candidate.

Essential skills:

  • Confident and empathetic communicator.
  • Excellent organisational skills.
  • Attention to detail and accuracy.
  • Developing and using searches within SystmOne clinical system.
  • Proficient in the use of Microsoft Office products.
  • Ability to self-direct tasks, use own initiative, to meet an overall goal.
  • Working in health, social care and other support roles in direct contact with people, families or carers (in a paid or voluntary capacity).
  • Experience of data collection and using tools to measure the impact of services.
  • Experience of working within multi-professional team environments.
  • Experience or training in personalised care and support planning.
  • Working with elderly or vulnerable people.
  • Experience of healthcare and complying with best practice and relevant legislation.
  • Supporting people, their families and carers in a related role.
  • Minimum of 1 year experience of working with healthcare professionals and/or previous experience in the NHS or social care or relevant field.

eQuality PCN was formed in 2022 and is a primary care network (PCN) serving a population of circa 41000 patients residing across Luton in Bedfordshire. The PCN strategy aligns with the needs of our population and the neighbourhood team model. As we seek to develop the services that are needed by our patient population, we are seeking to build a skilled and professional workforce.

Responsibilities:

eQuality PCN have an exciting opportunity for a passionate and dedicated clinical care coordinator to join our team. The role is to cover 37.5 hours a week and our service operates Monday to Friday, with flexibility of hours between 8am - 6.30pm.
Based within the eQuality PCN, formed by 4 GP practices (The Biscot Group, The Larkside Practice, Stopsley Village Practice and The Ashcroft Practice), the successful applicant will be placed within our supportive and proactive, friendly team. You will be working together to deliver the PCN’s key priorities: identifying patients with unmet clinical and care needs and organising their onward appointments, gathering data for clinicians and management staff, recruiting patients to our proactive care initiatives, and carrying out suitable clinical tasks.

Main duties include:

  • Proactively identify and support individuals, including the frail, elderly, and those with long-term conditions, providing co-ordination and appointment navigation for patients and their carers.
  • Conduct IT system searches and communication with clinicians and our multidisciplinary team.
  • Use SystmOne on a daily basis with confidence to run searches and keep care records up-to-date, including identifying patients with long-term health conditions and those requiring medication and disease monitoring and communicating with them to arrange recall and monitoring appointments.
  • Contribute to audits and data collection to aid evaluations of the PCN services will be needed.
  • Facilitate routine BP monitoring, phlebotomy, urine dips, BMI and NHS health checks, advising patient’s accordingly. Undertake clinical tasks as required. Support the clinical team with the management of long term conditions.
  • Adhere to infection prevention control policies, advise patients on diet, lifestyle and wellbeing.
