Essential qualifications, skills, and experience:

  • A GP whose name is included in the General Practitioner Register (GPR).
  • Included in the list of medical practitioners maintained and published by NHS England in accordance with section 91 of the NHS Act 2006.
  • Within 2 years of date of your certificate of completion of training(CCT), issued by the General Medical Council.
  • Highly developed interpersonal skills and ability to lead key meetings.
  • Excellent communication skills- written and verbal.
  • Able to drive and have use of a car.

ABOUT US

Green Vale Health Primary Care Network is a PCN in Central Bedfordshire. Our mission is to improve health and care and to meet everyday health and care needs for people by connecting primary care systems and using creative thinking to develop, improve and support great local services.
Our PCN is split over three sites, Flitwick Surgery, Marston Forest Healthcare (and Cranfield Surgery which is a branch surgery of Marston Forest Healthcare) in modern facilities. We have a combined list size of approx. 25000 patients with some 243 care home beds. We are not one of the larger PCN’s, but this smaller patient number allows us to provide first class healthcare and the ability to target diverse healthcare needs.
The sites are only a short distance apart (5 miles) but service the main population areas of Flitwick, Marston Moretaine and Cranfield as well as several surrounding villages in a rural setting, with Bedford, Milton Keynes and Luton only a few miles away.
Both practices within the PCN are training practices so we have the experience and knowledge to fully support a newly qualified GP with the necessary supervision and ongoing training needs.

Responsibilities:

ABOUT THE ROLE

Green Vale Health Primary Care Network has exciting opportunity for a general medical practitioner into our practice team on a fixed term contract until March 2025 with the possibility of an extension. The role is offered on sessional basis, 4 sessions per week between Monday and Saturday, on a rotation basis across the 2 surgeries - Flitwick Surgery and Marston Forest Healthcare that make up the PCN.

Responsibilities of the role will include:

  • Deliver high quality clinical care to the patients through face-to-face/online/telephone consultations, home visits, assessments, diagnoses, treatment plans.
  • Chronic disease management: To deliver care plans for chronic conditions and manage long-term conditions.
  • Preventative care and screening: Promote immunisations, screenings, and lifestyle interventions.
  • Referral and coordination of care: Refer patients to specialist services and coordinate care across primary, secondary, and tertiary sectors.
  • Health promotion: Engage in health promotion initiatives such as mental health awareness, smoking cessation, and weight management.
  • Public health and community engagement: Participate in public health initiatives to address social health determinants and inequalities.
  • Quality improvement: Take part in audits, research, and quality improvement projects.
  • Work with the PCN manager, practice managers and clinical directors to support the development, delivery and review of the surgery action plan.
