Description:
- Coordinate the Mental health teams rota monthly -Manage referrals and booking of mental health appointments, action clinic changes and cancellations, and communicate this with practices, patients, and the mental health team where appropriate -Support patients in navigating PCN and third sector services and make referrals where appropriate -Lead and coordinate the weekly Mental Health Multidisciplinary Team Meeting (MDT) -Create and analyse statistical reports to demonstrate the mental health teams effectiveness and enact positive operating procedural change based on any findings -Act as the point of contact for administrative and operating queries regarding the PCN mental health service and its auxiliary services -Maintain, and build new, positive working relationships with other primary care networks, our practices, and third sector organisations; sharing good practice and working collaboratively to the betterment of mental health provision in our area -Look for creative solutions or strategies to improve efficiency or the quality of service for our practitioners and patients -Work collaboratively with the operations team in leading or supporting on patient engagement events and/or pilot projects and initiatives -Design materials such as posters or leaflets to improve engagement of PCN or practice services, or to promote events -Keep up to date with your own CPD and complete all training relevant to the mental health care coordinator role -Look to promote proactive care with regards to mental health, identify relevant patient cohorts and gaps within the current service that could be improved -Use a range of digital software including Word, Excel, and Canva where appropriate
