Description:

JOB OVERVIEW:

The Nurse Coordinator RN in the Special Needs Primary Care practice provides guidance for patients and families seeking services by serving as an active liaison between patients/ families, medical sub-specialists, therapies, admissions, insurance companies and community medical providers within a family centered care model. They are responsible for assisting families in finding the proper resources to supplement medical care and quality of life, monitoring family and patient s understanding and compliance with treatment goals, interacting with social services and community agencies, and supporting family through provision of resources and support networks as appropriate.

QUALIFICATIONS:

Required:

  • Graduation from an accredited Nursing program
  • NJ Registered Nurse License
  • AHA BLS Provider Certification
  • Excellent verbal and written communication skills
  • Interpersonal, organization and planning skills
  • High degree of customer sensitivity
  • Cognitive analytical and problem-solving skills

Preferred:

  • BSN degree

SCHEDULING REQUIREMENTS:

  • Full-Time
  • 40 hours/week

Responsibilities:

ESSENTIAL FUNCTIONS:

  • Coordinates care for children across continuum from intake, outpatient and inpatient services
  • Coordinates communication between patient/family, internal and external service providers
  • Demonstrates an understanding of family centered care, patient s medical diagnoses, prognosis, progress and implications for family support, education and goal directed treatments and therapies
  • Provides triage for families to determine service needs
  • Reviews and organizes intake materials and consults with Practice team as needed
  • Engages in advocacy for patient, and foster family’s abilities to assume independent skills in seeking and obtaining supports and services for child
  • Demonstrates an understanding of the health care system, managed care and resource utilization
  • Refer patient and families to community resources to provide access to services such as financial assistance, legal aid, etc.
  • Obtains and manages outcome information for patients in care coordination program
  • Advocates for the unmet needs of patients / families across the medical, social and financial continuums
  • Provides necessary documentation to external agencies as needed
  • Effectively communicates financial and social needs, barriers, and issues to the multidisciplinary team, physicians and external agencies
  • Identifies families at high risk for child neglect or abuse and makes appropriate referrals
  • Be a resource for families to ease them through transitions and hardships
  • Completes proper documentation in a timely manner and provides necessary documentation to external agencies as needed
  • Acts to enhance understanding of CSH services and mission through development of relationships with community and government agencies, service providers and referral sources
