Description:

The Peer Mentor acts as a role model to their NDIS participants. The main objective for a Peer
Mentor is to ensure their participants are working towards achieving their NDIS goals. These goals
can be achieved through assisting participants to services, assist with and/or supervise participants to
engage in the community, as well as empowering and building participants’ capacity.
The NDIS is a major national reform with an insurance based approach. It is a new way of providing
support for eligible people with permanent and significant disability, their families and carers, via
individualized services based on a client’s level of need.

Responsibilities:

Community and home service

  • To support children and young adults in their home or community
  • Support children and young adults with a disability to be more active and independent at home and in

the community

  • Drive and or escort the participant to appointments or activities as set out in the treatment plan.
  • To adhere to the policies and procedures of Access Your Supports when working with participants.

Communication

  • Advise staff during or at the end of the shift of any changes in the health and wellbeing of the

participant, or of any incidents which require reporting.

  • In the case of emergencies follow emergency procedures including contacting Access Your Supports office staff immediately.
  • Consistently respond to emails and referrals from the Peer Mentor team in a timely manner.

Team work

  • Work as part of a team, cooperate and collaborate with the broader team and the organization
