As the Peer Navigator - LEAD, you will play a pivotal role in individuals’ lives, making a significant impact through:
- Provide on-going peer support that focuses on implementing harm reduction strategies aimed at helping individuals minimize their health risk
- According to program guidelines, consult with case management staff, Health HUB staff and/or participant’s providers on progress of service/care plan services
- Work with Syringe Exchange Program staff to facilitate access to Harm Reduction supplies as needed
- Conduct prevention related harm reduction/risk reduction interventions with participants
- Utilize supervision to discuss collective challenges of team members, and assist supervisor with formulating and implementing a plan to resolve barriers
Peer Navigator - LEAD Minimum Salary: $38,000
Peer Navigator - LEAD Maximum Salary: $40,000
Peer Navigator - LEAD Schedule: Monday - Friday 8:30 - 4:30
Qualifications:
- Associate’s Degree in Health Education or Human Services
- 1-3 years of experience working with a diverse population
- CRPA (Certified Recovery Peer Advocate) Certification preferred
Responsibilities:
