Description:

Hope Haven is an accredited resource for disability services and advocacy. We offer service programs and support to meet a broad range of needs for children, adults, and families – from physical, mental, and intellectual to residential, vocational, and spiritual.

POSITION OVERVIEW:

This on call position consists of providing community-based Peer Support services in homes and/or community settings of individuals served as well as in a recovery center. Individuals will be responsible to meet all contact and documentation requirements of Medicaid services and other funding entities.
This would be roughly 8-9 hours per week

PHYSICAL REQUIREMENTS:

  1. Physical ability to drive or able to access transportation for both self and consumers.
  2. Physically able to get out into community locations with consumers.
  3. Ability to perform light janitorial duties
  4. Ability to enter accurate documentation notes into the computer.
  5. Worker must be able to visit and work in home environments that are possibly in disrepair, not maintained, unclean and/or un-kept, or handicap accessible. Offensive odors and animals may be present. Unobstructed face-to-face contact is essential and the worker must not have allergies that would prohibit working in homes or offices or require wearing an allergy or other type air filter mask.

QUALIFICATIONS:

  1. Current or past recipient of mental health services; presently stable in mental health symptoms.
  2. Have taken a qualified 40-hour course in Peer Support Services and/or be willing and able to take this type of training upon hire.
  3. Knowledge and understanding of “recovery” model of service delivery.
  4. Excellent telephone skills.
  5. Strong interpersonal/counseling skills.
  6. Willingness to work as a “team” member.
  7. Ability and willingness to learn computer based data entry.
  8. Ability to organize work and carry out written and oral directions.
  9. Up-to-date physical with approved working health status and negative drug screening at time of hire.
  10. Clearance from a criminal background and abuse reporting check at time of hire and Department of Motor vehicle annually.
  11. Must be able to attain a Class D, Limited Chauffeur’s License with endorsement 3 prior to beginning work and must be insurable by our insurance company.

Responsibilities:

  1. Provide consumers who have psychiatric disabilities with supportive contact sessions that are conducted via face-to-face and phone consults. Face to face meetings may be provided in a person’s home, in a community setting, or in a drop in center.
  2. Create a safe and inviting place for people with psychiatric disabilities to learn skills that assist them in integrating into their community as well as building on their recovery.
  3. Work as scheduled which includes arriving on time for shifts and/or individual appointments, and keeping appointments or assignments.
  4. When doing drop-in center shifts, you are expected to actively engage with participants throughout the shift.
  5. Interact with all persons served in a positive manor.
  6. Provide support to people as the need arises to identify and/or obtain community based resources.
  7. Help person’s develop individual coping plans and report any crisis/emergencies.
  8. Complete service delivery and documentation as required by the funding stream paying for the service provided.
  9. Participate in Peer Support staff team meetings, supervision, and training as scheduled
  10. Have knowledge and understanding of program standards and guidelines.
  11. Comply to Hope Haven’s Policies and Procedures.
  12. Responsible to maintain positive internal and external relationships with customers.
  13. Follow all safety procedures and promote safety measures among the team and clients.
  14. Other duties as assigned by your supervisor or management staff.
  15. Regular and reliable attendance is required.
  16. When applicable, participate in the interdisciplinary service plan meetings of the person(s) you are serving.
