Description:

You will be expected to liaise with and work alongside other staff, agencies and stakeholders involved in the discharge process (including Responsible Clinician and other hospital staff, community Care Coordination clinicians, community forensic teams, local mental health teams, substance misuse specialists, housing providers, carers, 3rd sector stakeholders). To share personal experiences of recovery and coping to build connection and inform support. To use active listening skills to help people develop self understanding and identify their personal recovery goals. To support others in achieving goals they have identified themselves and to role model recovery, personal responsibility and self awareness.
A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010
