The NHS Long Term Plan for Cancer states that where appropriate every person diagnosed with cancer will have access to personalised care, including a full personal needs assessment, an individualised care plan and health and wellbeing information and support.
The post holder will facilitate the local delivery of the Personalised Care and Support agenda across all directorates in Sheffield Teaching Hospitals involved in the care and treatment pathway for patients with cancer from diagnosis, surgery, non-surgical and follow up care.

Support the Trust Lead Cancer Nurse to facilitate and coordinate the delivery of the Personalised care and support agenda.
To work in partnership with CNS and cancer nursing teams across the cancer MDTs to identify the skills and knowledge of teams and individuals to deliver Personalised Care
Work in partnership with the CNS and cancer nursing teams and integrate new ways of working on order to deliver Personalised Care
Work collaboratively with equivalent peers across the Cancer Alliance footprint and link to other aspects of the personalised care initiatives are prioritised at system level.
Work closely with the other Personalised care Leads across the Alliance and provide a central point for communication between all partner organisations.
Share learning and good practice and to optimise opportunities for cross boundary/cohesive working and standardisation where possible. This will include attendance at the Cancer Alliance Personalised Care and Support Programme Steering Group.
Provide overall responsibility for the successful initiation, planning, design, execution, monitoring, controlling the Personalised Care and Support Agenda across Sheffield Teaching Hospitals.

This vacancy is advertised on a fixed term/ secondment contract basis for a period of 24 months due to the post being externally funded. The fixed term contract status of the post holder will be reviewed in accordance with the Trust’s legal obligations regarding fixed term contracts taking into consideration the ongoing source of the external funding and the requirement for the post to be maintained.
