Description:

INTERNAL/EXTERNAL POSTING

Under the direction of the Director of Primary Care, the Primary Care and Shelter Supervisor is responsible for supervising the daily operations and service provision coordination of the Primary Care and Shelter Health program. They are also responsible for providing clinical and crisis leadership to the front-line service delivery staff.
The incumbent supervises the ongoing provision of specialized, comprehensive community-based treatment, primary care support to Windsor-Essex County area individuals who are experiencing a mental health and/or addictions crisis, who are homeless, and who are at risk of adverse health outcomes. The position accomplishes this in consultation with the Program Director through program development, referral liaisons and referral pathways, identifying and measuring outcomes, community resource partnership building and continuous quality improvement initiatives.
In addition to program coordinating activities and operations leadership to the clinical team, the position oversees front line services within the framework of a client directed clinical/rehabilitation case management model. This model includes a continuum of functions, including assertive outreach, building therapeutic relationships, developing care plans with clients, client and systems advocacy, symptom management, life skills teaching, trauma-focused supportive counseling, motivational interviewing, family support, and crisis intervention, including wellness checks, crisis counselling, and risk mitigation plans.
This full-time position requires the incumbent to provide occasional coverage on the afternoon shift . See below hours of work for details.

EDUCATION & SKILLS:

  • Requires the knowledge and skill acquired through the completion of a four-year degree in a discipline deemed relevant by the employer (i.e. BSW, BScN) from a recognized post-secondary institution combined with significant case management experience or completion of a Registered Practical Nurse diploma from a recognized post-secondary institution combined with two years of leadership experience and significant case management experience.
  • Registered member in good standing of a professional college deemed relevant by the employer such as the Ontario College of Social Workers and Social Service Workers (OCSWSSW) or the College of Nurses of Ontario.
  • Three (3) years recent, relevant case management and outreach experience working with vulnerable populations who are experiencing concerns including but not limited to: homelessness, addictions and mental illness.
  • Experience with the applicable mental health legislation.
  • Demonstrated leadership and/or supervisory experience. Completion of leadership courses is an asset.
  • Demonstrated ability to work unsupervised and manage a diverse and heavy workload.
  • Demonstrated problem solving skills to manage contentious or sensitive situations with diplomacy and tact.
  • Bilingualism in both official languages is an asset for all agency positions.
  • Valid Ontario Driver’s License and own means of transportation are required.
    Hours of Work: Flexible hours are required to meet service needs. This full-time position requires the incumbent to provide occasional coverage on the afternoon shift. The primary hours of work are 8:30am - 4:30pm with the occasional afternoon shift (approximately 12pm - 8pm once per week). Thirty-Five (35) hour work week. There will be five seven-hour shifts with a one-hour unpaid lunch.
    Salary range: $64,822 – $79,500 annually
    Compensation within the range will be based on the experience of the incumbent.

Responsibilities:

  • Responsible for the day-to-day operations of Primary Care and Shelter Health, including, immediate risk assessment and mitigation, SOR reporting, task delegation, primary consultant with the Program Director, team meeting coordination and facilitation and follow up, providing program updates to the team, and coordinating Facility Services and Information Technology Services supports to address Program needs.
  • Responsible for day-to-day implementation and management of the clinical service delivery system for the client service population.
  • Provides clinical leadership, education and coaching to the front-line team. This includes, but is not limited to training, orientation and support to new staff members, volunteers, and students assigned to the program and substitutes for and assists other agency staff where skills are applicable. Provides ongoing support and assists in evaluation and skill development of staff, students and volunteers.
  • Manages direct reports including but not limited to: delegation of work, hiring, day-to-day supervision, managing performance, coaching, conducting performance appraisals, etc.
  • On-site, day-to-day supervision of the staff, including providing education/training, direction, support, and other relevant educational resources as it relates to the program’s service mission.
  • Developing and fostering relationships with the H4 team, City of Windsor, WPS and other partners by facilitating communication and providing ongoing information/education and program updates regarding programming.
  • Developing, evaluating, improving and expanding programming while participating in creating critical safety protocols. Works collaboratively with the Program Director on program evaluation and change management, developing recommendations and strategies for overall program improvement.
  • Client and Systems advocacy and linkage to resources: Assists clients to access information as well as internal/external resources, services or supports which are deemed necessary. Where resources do not exist or they are inadequate, advocates within system to develop or improve essential services and resources for clients.
  • Provides daily on-call clinical/operational support for the Program between the hours of 1600 and 2330 including weekends and statutory holidays.
  • Provides additional support and direction to staff as required.
  • On-call responsibility during Vacation time, sick days and other instances where the Program Supervisor would be offline would be rotated to the Program Director or designate.
