QUALIFICATIONS:
Required:
- Doctor of Pharmacy from an ACPE accredited school required,
- Current NJ Pharmacy License required,
- NJ Board Registration required
Preferred:
- Minimum of one year accredited pharmacy residency strongly preferred,
- Previous hospital experience strongly preferred,
Responsibilities:
- Reviews physician orders for Appropriateness of drug, dose, frequency, and route of administration, Therapeutic duplication, Age appropriateness, Allergies or sensitivities, Current or potential impact as evidenced by laboratory values, Food drug and Drug drug interactions, Other complications
- Compares new orders with current patient specific information and resolves all drug related problems, in conference with prescribing physician when necessary,
- Provides pharmacy documentation of interventions,
- Communicates and confers with both nursing staff and patients in order to obtain relevant clinical information,
- To provide information to support the nurses responsibilities for administering medications, monitoring drug therapy, and providing patient education,
- When appropriate in order to obtain relevant clinical information,
- When possible, and when appropriated, educates patients regarding medications prescribed at discharge
